Practice Management on Embodia - Part 1: Charting

If you use the questionnaires feature on Embodia to send your patients outcome measure or intake forms, you can connect a completed questionnaire to a chart entry. To do so, from the chart entry page, click on the Other actions button, and then on Connect questionnaire.

In the pop-up form, select the completed questionnaire and then click on Submit. The selected questionnaire will be added above the chart items in the chart entry.
